Integral Care approach
Our Care Managers and Clinical Nurses will be on hand to assess, plan and upskill our highly motivated support workers to implement all your care needs. We aim to ensure all service users are cared for in their own home or in named venues within the wider community i.e school, college, social groups and day centres.
Our approach is client-specific and nurse-led, meaning all service users are allocated a nurse to provide clinical expertise and assist our support workers to deliver day-to-day complex care and carry out clinical tasks required to meet your needs.
Our expert team of nurses assesses each of our service users clinical and social requirements, to develop a robust care plan along with completing risk assessments as safety is paramount.
We endeavour to maintain a high standard in promoting a seamless service that is why we take the time and give you the opportunity to meet support workers that are highly skilled and where possible share the same interests and personality matching - because we know how important it is to have the right person caring for you and working with you.
Management will conduct quarterly supervisions with all support workers to ensure that you are getting the care you need and that the support worker is empowered to do their job. Regular spot checks are carried out during their shift and these are arranged with you for quality assurance
We evaluate our care based on service users’ feedback, staff satisfaction and via our constructive working relationships with ICS' and Social Services alike.
We take a holistic, family centred approach to the way we work with our service users and how we deliver our service. All cares provided are specifically designed to meet the needs of everyone directly using our service.
