Integral Care provides nurse-led complex care for children, young people and adults with a wide range of health and support needs across London and Kent. Our service is designed for people who require safe, reliable and clinically informed care in their own home, school, college, workplace or community setting.
We support individuals with long-term conditions, acquired injuries, congenital conditions, physical disabilities, neurological needs, respiratory needs, palliative care needs and other complex presentations that require skilled monitoring and a planned approach to care.
Our service supports planned discharges from hospital, transitions from children's services to adult services, respite arrangements, transitions from another provider, emergency cover and ongoing packages of care. We focus on careful mobilisation, clear communication and practical preparation to minimise disruption and support continuity of care.

We have expertise in meeting the care requirements of ICBs by providing client-specific waking night 1:1 or 2:1 up to 24-hour complex health care or nurse-led support, with a mobilisation rate of 1-2 weeks set-up time.
